Transitional Care

The Road to Transition

The Transitional Care Service aims to provide a seamless transition for young people with life limiting conditions from adolescence to adulthood. This is achieved through working with in-house services, other healthcare professionals and external agencies.

The aim of the service is to provide specialist and age appropriate respite care in a Day Hospice setting; to support each individual to achieve their maximum potential in terms of education, health, physical, psychological and social development.

The Young People’s Hub in the Hospice will facilitate this development through individual care with the support of appropriately qualified nurses, the Family Support Service and volunteers. Both services and staff will respond in an appropriately sensitive way which encourages engagement and provides high quality support for young people and their families.


Who do we help?

We offer co-ordinated transition service for young people aged 16-24 years with a life limiting condition. Some of these young people may eventually require palliative care, which the service can provide. The service aims to provide quality care, advice and professional assistance for the families and siblings of young people in transitional care.

Referral criteria will include life limiting conditions, for example:

  • Duchene Muscular Dystrophy
  • Battens’ Disease
  • Severe Cerebral Palsy
  • Brain or spinal chord injury
  • Cancer
  • Severe cardiac conditions


How do we help?

The service provides a range of support to patients and their families, including:

  • Respite care in a Day Hospice setting
  • Psychosocial support for young people and their families
  • Youth groups, giving young people the opportunities to meet with each other
  • Support for siblings
  • Bereavement support for family members and loved ones.


The service will be delivered within the bespoke young people’s facility at The Shakespeare Hospice. Young people will have access to a communal meeting space, quiet room, one to one therapy room, kitchenette and a wet room with hoist facilities.
