Hospice at Home Service

About the Service:

Our dedicated team of experienced Registered Nurses and Health Care Assistants, who have received specialised training in all aspects of palliative and end of life care, provide compassionate support, advice and nursing care for patients at home.

The team work as part of the Integrated Health Team, which includes District Nurses and GP’s, to ensure our patients receive the full support and advice required to enable them to remain at home.

Our service is nurse-led and available to anyone over the age of 18 years who is registered with a South Warwickshire GP.

What can you expect from us?

  • Experienced nursing care for patients approaching the end of their lives, who wish to remain at home, surrounded by those who are important to them
  • Emotional and psychological support for patients and their families and carers
  • Expert knowledge in assessment and management of physical symptoms, including administration of medications
  • Whilst we cannot prescribe medication we will work closely with the appropriate professionals to ensure that your medications are effective for your symptom management
  • A plan of care designed around your needs and wishes
  • Advice and support 365 days a year
  • Specialist care and advice to prevent hospital admission
  • Service provided from 8.30am to 10pm with an emergency contact telephone number available for overnight advice and practical support from a qualified nurse
  • Advice about other beneficial services which are available
  • Bereavement support

How to access our services

If you would like to refer yourself or a family member, or have any questions about our service please contact us on 01789 299426, between 8.30am and 10pm, 7 days a week.

Professionals can make a referral on our Single Point of Access telephone line, to support a faster referral process, as we acknowledge the potential time pressures that many are faced with.  Please call 01789 333 139.  There is a facility to leave a voicemail with your contact details and we will get back to you at the earliest possibility.  The line is manned from 9am to 5pm Mon to Fri and messages will be picked up until 10pm 7 days a week.  Any message left after 10pm will be actioned the next working day.
